File of Life Program

 

University Hospital EMS is proud to present the File of Life program. This program, created by the File of Life Foundation, is designed to provide emergency services with information on patients who would otherwise be unable to communicate, due to medical condition and language barriers, among other reasons.

The form (see link below) is to be filled out and placed on your refrigerator, or be accessible for the patient to present to emergency responders. The form should be updated as information changes, and you are welcome to download this form as many times as you need, in order to keep the information up to date.

Maintaining an up-to-date form is essential for providing the best possible patient care. The information on this form can help responders understand a potential medical condition, and may also be the only way to communicate with a patient who speaks a foreign language.
